Where to submit a request

You can submit a request for information under Act No. 106/1999 Coll., on free access to information:

• by email to [email protected],
• through the data box (ID: n233fup),
• by post to our registered office: Zdravá Sedmnáctka s.r.o., Na Chobotě 1420/16, 163 00 Praha 6 – Řepy,
• in person at the reception of the centre.

Anyone can make a request and does not have to give a reason. You will find what the request must contain, the deadlines for dealing with it and what to do if you disagree with how it was handled in the Public information page.

Payment of costs

We do not charge any fee for providing information.

Requests we have received

We publish the requests we have received and our replies. The personal data of the applicants is anonymised.

20 February 2025 – stocktaking of assets

• Applicant: a member of the assembly of the Prague 17 Municipal District.
• What the applicant requested: the plan for carrying out the stocktaking of the company’s assets, the internal rules for carrying out the stocktaking, the stocktaking carried out as at 31 December 2024, the accounting records for it and the trial balance of turnovers as at 31 December 2024.
• Request: Request for information of 20 February 2025 (PDF, in Czech, anonymised).
• Reply: sent on 7 March 2025 by data message to the applicant and for information to the Prague 17 Municipal District. It had eight attachments in PDF format (among them the stocktaking guideline, the stocktaking record and the appointment of the stocktaking committee) and six spreadsheets.
